looking for a high-performing Home Health Physical Therapist (PT)

PTO. Your Mission As an Archer Health PT, you are the architect of
a patient’s recovery. You will provide skilled therapy services in
the most meaningful environment possible—the patient's home. Key
Responsibilities: Comprehensive Assessment: Evaluate therapeutic,
rehabilitative, and functional status; assess muscle strength,
mobility, gait, and ROM. Plan of Care: Develop and implement
individualized treatment plans in collaboration with physicians and
our interdisciplinary team. Home Environment Safety: Evaluate the
home for hazards and recommend modifications or assistive devices
to ensure patient safety. Education: Instruct patients and
caregivers in therapeutic exercises and the use of appliances
(orthotics, prosthetics, etc.). Supervision: Oversee Physical
Therapy Assistants (PTAs) and Home Health Aides (HHAs) to ensure a
cohesive care journey. Clinical Excellence: Maintain accurate,
timely documentation (point-of-care) that reflects the high

clinical results. Qualifications: Education: Graduate of an
accredited Physical Therapy program (DPT preferred). Licensure:
Current, unrestricted California Physical Therapist License.
Certification: Current BLS/CPR Certification. Experience: Previous
home health experience is highly preferred; experience with
high-acuity patients is a plus. Mobility: Valid CA Driver’s License
with a clean driving record and reliable transportation.
Communication: Exceptional interpersonal skills and the ability to
