Job Description:
A company is looking for a Social Worker for the Quality
Operations Call Center - Remote.
Key Responsibilities:
Conduct telephonic outreach to meet quality metrics and production
targets
Perform comprehensive assessments and document findings in
compliance with department requirements
Coordinate community resources and provide support to improve
Social Determinants of Health for members
Required Qualifications:
Bachelor's in Social Work (BSW) from an accredited program
Active, unrestricted Compact Licensed Social Worker (LBSW) in the
state of work
2+ years of experience as a social worker in a healthcare
setting
Experience addressing Social Determinants of Health (SDoH)
Proficient in PC software skills such as Microsoft Word, Excel,
PowerPoint, and Outlook
