Position Summary:
We are seeking a Manager of Care Management (RN or MSW) to lead and manage a high-performing team of Case Managers and Social Workers at our Fullerton campus. Reporting to the Director of Care Management, this individual will oversee daily operations, implement effective care management strategies, and ensure patients receive optimal care and resource support through the continuum of care.
Key Responsibilities:
Lead and manage a team of RNs and Social Workers within the Care Management Department.

Oversee and improve daily operations, ensuring compliance with clinical standards and regulatory requirements.

Coordinate with interdisciplinary teams to facilitate comprehensive discharge planning and resource utilization.

Advocate for patients to access timely care and support services across ambulatory and acute care settings.

Support department initiatives through strong project and operational management.


Required Qualifications:
Bachelor's degree in Nursing (BSN) or Master's degree in Social Work (MSW)

Minimum 3 years of recent experience in an ambulatory or acute care setting

Minimum 1 year of leadership experience managing clinical teams or healthcare operations

Strong knowledge of case management principles and healthcare delivery systems


Preferred Qualifications:
Master's degree in Nursing, Health Administration, or related field

Case Management Certification (CCM or ACM)

3 years of management experience in a healthcare setting
