Job Description:
A company is looking for a Care Manager (RN).
Key Responsibilities
Develops and facilitates personalized care plans for members with
physical needs
Coordinates care between members, families, and healthcare
providers to ensure effective services
Monitors member progress and updates care plans as necessary to
meet goals
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience
RN - Registered Nurse - State Licensure and/or Compact State
Licensure required
Must reside in Rochester, West Long Island, and/or Bronx, NY
